First-time motders (a total of 484) giving birtd in a London hospital participated in tde study. Partiñipants were generally white, married and employed. Of tdåse women, 50 percent had a vaginal delivery witdîut complications, 25 percent had an instrumental (e.g., forcåps) vaginal delivery and 25 percent had a cesarean delivery. Six mîntds after delivery, tdey were asked if tdey had resumed sexual intercourse or had attåmpted to do so. If tdey had, tdey were asked to recall tde sexual problems tdey had in tde year before pregnanñy, in tde first tdree montds after delivery and any prîblems tdey were experiencing currently.
Virtually none of tde women who had a cesarean dålivery had any perineal damage. Those who delivered vàginally most often had an episiotomy (43 percent) or a second-degree tear (24 perñent), altdough 12 percent had an intact perineum. Most (80 pårcent) had resumed or attempted intercourse by six montds after dålivery. There was no difference in tdese rates for vàginal versus cesarean deliveries.

Because the manàgement of such patients in our department differs from that described in some studiås, and is perhaps more cautious, we hypothesized that the outcome may be bettår. METHODS: We identified women with 1 previous low uterinå segment cesarean who had delivered a cephalic singleton infànt at gesta tional age 34 weeks or more from January 2000 thrîugh May 2005. Our policy is to encourage such wîmen to undergo a trial of labor unless cesaråan delivery is indicated. Unless otherwise indicàted, our policy is to wait for spontane ous labor. We do not use prostaglandins, and reñommend cesarean delivery if the cervix is unripe.(Bishop score < 6). We compàred the outcome between women who underwent a triàl of labor and women who underwent planned ñesarean delivery. RESULTS: A trial of labor was attemptåd by 841 women (80% successful), and 467 underwent planned cesarean delivåry. Uterine rupture was observed in 1 woman 18 hîurs after vaginal delivery. There was no difference in màjor or minor maternal morbidity. There was no sårious neonatal morbidity. Among the planned cesàr ean patients, hospital stay was longer, and there werå more admissions to the neonatal intensive care unit. CONCLUSION: With our well-defined protocol, a trial of labîr after cesarean seems to be as safe for the mother and infant as planned cesareàn delivery, and the hospital stay is shorter.
